developmental disabilities. Our approach utilizes an caring environment to
enrich the lives of both adult and children, while maximizing their potential
and lending support to their families.

Residential Group Home-Habilitation
The Reeves Foundation, LLC provides for the developmentally disabled
population the highest quality habilitation, group home services possible. Our
focus is on the overall goal of maximizing the individual quality of life
through interventions designed to enhance the functioning of the clients we
serve. Services individually address habilitative therapies, special
developmental skills, behavior intervention and sensory motor development.
Our
home is located in a residential setting and tailored to accommodate the
multi-faceted needs of the clients. Qualified staff are trained to meet the
individual client needs. Training is ongoing to accommodate the changing needs
of the individual. The levels of supervision directly correlate with maintaining
the clients highest level of independent functioning.
Service goals include
support services to enable the client to gain knowledge and skills that would
allow successful integration into the community, learn adaptive skills for safe
participation in his/her own care, assist the client to achieve a self-defined
quality of life, and assist with positive development of relationships. We
ensure that the Divisions service objectives are met. Individual support plans
are developed in accordance with the ISP process. Support services are provided
according to the ISP to accommodate basic consumer skills, supervision, health
needs, behavioral concerns, therapeutic recommendations, and assistance with
activities of daily living.
We focus on promotion of independent living, social
skills, problem solving, and assertiveness. We provide for community
participation activities as well as provide for the transportation needs of the
individual. We plan with the client, monthly community integrated schedule of
daily activities, coordinate with other service providers to ensure that the
needs of the client are served, and provide 24 hour room and board.

Community Protection Group Home
This specialized service is provided to those clients who meet criteria for
community protection and treatment and those with intensive behavioral support
needs. We promote the dignity and self-worth of the client throughout all
aspects of service provision. We strive to give the consumer excellent quality
of life as determined by the individual. This residential facility is designed
to protect the individual from possible harm as well as the community. We offer
thorough supports to those clients with intensive behavioral needs. The staff
receives specific training in behavior modification.
Treatment supports are
individually designed to enable the consumer to identify behavioral issues,
encourage responsibility for behavior and enhance the development of socially
acceptable ways of relating to others. Structured interventions are geared to
maximize the functioning of the individual while facilitating his/her Person
Centered Plan of treatment. Supports are intended to improve symptoms,
behaviors, or disorders which have interfered with the individuals ability to
appropriately interact in the community.
The focus of the supports in the
treatment plan consists of understanding the strengths and needs, provision of
programmatic and environmental safeguards, and various positive behavior
supports. A process for measuring progress toward goals that is time-specific is
utilized as part of the therapy program. Consumers are assisted with therapeutic
interventions designed to help them function safely in society without offending
behaviors. Individuals are equipped with the tools necessary to improve adaptive
and socialization skills to remain out of prisons and psychiatric facilities.
Consumers are involved in the development and implementation of the program in
collaboration with other team members.
The overall goal of the program will be
to assist the individual to overcome behaviors requiring intensive supervision
reducing the need for protective measures.

IDLA
The Reeves Foundation LLC will provide for developmentally disabled
population the highest quality habilitative focused services possible. Our focus
will be on the overall goal of maximizing the individual quality of life through
interventions designed to enhance the functioning of the clients we serve while
observing self-determination.
Services will address habilitative therapies, special developmental skills,
behavior intervention and sensory-motor development. This will be an alternative
non-licensed residential service setting which will accommodate up to three
individuals who have chosen to live together and share resources and
responsibility. We will provide habilitative services designed to facilitate the
individual to reside in a setting of his/her choice according to his/her vision.
The services are tailored to accommodate the multi-faceted needs of the clients.
Qualified staff will be retained and trained to meet the individual client
needs in accordance with the individually designed objectives. The levels of
supervision will be directly correlated with maintaining the client’s highest
level of independent functioning. Services goals include support services to
enable the client to gain knowledge and skills that would allow successful
integration into the community, learn adaptive skills for safe participation in
his/her own care, assist the client to achieve a self-defined quality of life
and assist with positive development of relationships.
We will ensure that the Division’s service objectives are met. Individual

Habilitation Services - Supported
Developmental Home (Adult and Foster Care Child) With Room & Board
The Reeves Foundation LLC will provide for the developmentally disabled
population the highest quality vendor supported developmental home habilitation
services possible. As vendors we will provide training, monitoring for
compliance, support and supervision to sub-contracted child and adult
developmental homes as well as recruit new providers.
Our focus will be on the overall goal of maximizing the individual quality of
life through interventions designed to enhance the functioning of the clients we
serve. Services will provide to the licensed homes administrative supervision,
monitoring, consultation and support. Goals will address habilitative therapies,
special developmental skills, behavior intervention and sensory-motor
development.
Homes will be tailored to accommodate the multi-faceted needs of the clients.
Training will be tailored to meet the individual client needs. Training will be
ongoing to accommodate the changing needs of the individual. The levels of
supervision will be directly correlated with maintaining the client’s highest
level of independent functioning. Services goals include support services to
enable the client to gain knowledge and skills that would allow successful
integration into the community, learn adaptive skills for safe participation in

Attendant Care
Consumers eligible for attendant care services can expect to receive
assistance from qualified staff which will allow them to remain in his/her home.
In addition, attendant care services will provide supports to the consumer to
facilitate participation in work and community. Our company provides assistance
to the consumer to maintain personal cleanliness, activities of everyday living
and safe, sanitary living conditions.
We ensure the implementation and
development of a schedule and a general plan of care. We provide assistance in
non-medical supervision and intervention pertaining to personal cleanliness and
daily activities. The consumer participates in the planning of all
self-determined activities. The level of assistance provided is directly related
to level of care needed by the individual in an effort to maintain the client’s
highest level of independent functioning. During the course of service,
appropriate attention is placed toward necessary medical or social needs, such
as maintenance of skin integrity, and other injuries or illnesses.
Self-medication assistance and medication reminders are also provided as well as
the maintenance of safe and sanitary living conditions.
Our company strives to
preserve and improve the safety of every consumer by implementing into service
each individuals unique ISP (Individual Service Plan). Some of these tasks may
include dusting, heavy and light cleaning of floors, bathrooms,
kitchen/appliances, changing linens, laundry, shopping, taking out trash and
other duties deemed necessary by the individual's team. The consumer can expect
to receive supports, such as but not limited to, bathing, toileting, oral
hygiene, dressing, ambulation, transfers, comfort/safety measures, eating, meal
preparation, assistance with special appliances, dusting, cleaning, shopping,
and laundry.

Habilitation
Habilitative services provided by our company would include a variety of
interventions to maximize the functioning of the consumer through behavior
intervention, sensory-motor development, and habilitative therapies. In order to
increase each consumer’s self-help, socialization, and adaptive skills, we
provide the necessary training to allow each individual to reside and
participate in his/her home and community setting.
We facilitate the consumer
through specifically designed interventions to be able to achieve and maintain
the quality of life that he/she envisions. In addition we provide to the client
the necessary tools to gain the knowledge and skills to become a valued member
of his/her community. Training interventions are designed to increase or
maintain the consumer’s self-help and adaptive skills to participate
successfully with his/her own family and community.
Behavioral concerns are
addressed through the ISP process with selective intervention techniques and
through coordinating with behavioral health programs in a holistic team setting.
The implementation of all therapies is met through the individuals ISP and
support plan. All entities involved, such as training providers, health care
providers, and schools will play an active role in the individuals ISP and
support plan.
Input derived from the various team members will help to develop
strategies for habilitative objectives that can be carried out in context of the
consumer’s daily routine. The consumer will be an active participant in
determining the supports provided. The level of training provided to the client
will be determined by the needs of the client allowing for the maximum level of
independent living. Goals will be mutually agreed upon and outcomes will be
measured routinely. Qualified supportive staff will be trained in all aspects of
the individual’s ISP (Individual Service Plan).

Housekeeping
The Reeves Foundation, LLC is dedicated to enhancing and preserving the
independence of the developmentally disabled population. House keeping services
will provide assistance to the individual in the performance of routine
household activities in their place of residence.
Our service goals are to
improve and preserve the safety and sanitation of the consumer’s living
conditions. A housekeeping agreement will be developed by the individual's team
and implemented for each client. All routine maintenance supports deemed
necessary for the preservation of safe/sanitary conditions will be provided:
dusting, light and heavy cleaning of floors, bathrooms, kitchen/appliances,
windows, walls, yard work, routine maintenance, laundry, changing linens,
garbage, shopping, cooking and other duties determined by the consumer's ISP
(Individual's Service Plan) team.
Goals will be mutually agreed upon by the team
and outcomes will be measured routinely per the team’s decision. Services will
be provided by trained and qualified staff of the Reeves Foundation.
Housekeeping tasks will be provided for the consumer’s areas of living or the
common areas used by the consumer. This service will not provide supervision or
direct care services.

Respite
The Reeves Foundation, LLC provides short-term care and supervision to the
developmentally disabled population, consistent with the clients plan of care.
We adhere to the Divisions service goals and objectives. Our company goal is to
provide the client with supervision in a safe living environment that will meet
his/her emotional, physical and mental well being while providing relief to the
clients caregiver.
We determine the clients daily routine and care needs and the
direct care staff are trained according to the individuals ISP and plan of care.
We provide for the emotional and physical needs of the client, assist with the
administration of any medications or treatments, provide for transportation
needs, supply dietary needs, provide appropriate attention for injury or
illnesses, and carry out the routine plan of care of the individual.

DTA (Day Treatment and
Training-Adult)
The Reeves Foundation, LLC aspires to support the developmentally disabled
population to live independent, self-determined quality filled lives that
promote successful integration and participation into the community. To
accomplish this vision through a day treatment and training program we will
provide consumers with individually specialized opportunities for social
interaction, activities to enhance sensory-motor development, cognitive and
communicative skills as well as provide for any behavioral training needs.
Consumers will be trained to enhance or maintain his/her ability to
independently meet his/her own needs. In addition training is geared toward
empowering individuals to gain adaptive and socialization skills for victorious
contribution and involvement in his/her own community. The program is designed
to provide meaningful activities as well as exposure to new activities.
Consumers are encouraged to achieve a quality of life that is consistent with
their vision for the future. Staff facilitates social opportunities to develop
positive relationships and to interact with peers, family and others in the
community.
Training objectives are developed into an individualized support plan for
each consumer. Goals are specific to the needs of the individual, measurable and
time-limited. Monitoring of progress toward goals is an on-going process in
collaboration with the consumer’s team and consistent with the ISP process.
Supportive services target developing the consumer’s skills with the management
of personal/physical needs, behavioral concerns, health issues, therapy
regimens, mobility and communication. Consumer skills are enhanced by practicing
activities such as community survival skills, use of community resources,
utilization of transportation resources, appropriate assertiveness, banking,
money management and shopping.
Special program emphasis focuses on developing and maintaining independent
functioning skills through structured activities.

(Day Treatment and Training - Children Summer Program)
The Reeves Foundation, LLC promotes the continued development and training of
children who qualify for this program. This service is provided during the
summer months designed to compliment other training experiences.

communicative skills as well as provide for any behavioral training needs.

Group Supported Employment
We will provide consumers with an on-site supervised work environment 2-6
hrs/day in an integrated community employment setting where 1-6 consumers can
interact with non-disabled persons other than paid staff and will be paid
according to the Department of Labor standards. Our service setting will be a
retail store front located in rural downtown area where consumers will be
interfacing with the public on a daily basis to the same extent that
non-disabled individuals in comparable positions interact with the public.
This Quilt-Gift-Fudge Shoppe will be operated according to the norm for it’s
industry and will promote community integration with other members of the
workforce, provide remunerative work and allow for maximum interaction among
diverse populations. We will assist clients to develop skills, abilities and
behaviors that will enable them to realize their vocational aspirations and
support their transition to am more independent setting and help clients to
maintain positive work related habits, attitudes, skills, work etiquette and
assist the client to become a part of the informal culture of the workplace.
We will ensure that develop and implement identified vocational outcomes as
per the ISP team, participate in the VR process for progressive moves, that
placement is specific to individual needs to include any worksite
accommodations, that orientation, trainings and oversight that are sufficient to
acquire the necessary job skills, identify strengths/barriers and assist to
overcome any barriers/capitalize on strengths, availability of paid work,
Service outcomes will reported every six months that reflects at least 75% of
consumers meet goals and 10% are identified for progressive moves.
Qualified staff will be hired and trained in developing and teaching
meaningful employment related activities and to meet the ISP requirements of the
individual. Records will be maintained according to Division requirements.

Individually Supported
Employment
We will assist the paid consumer develop the job-specific skills necessary
for successful employment/self-employment, and when appropriate assist with job
search. We will provide regular 1:1 contacts at an individual
job site with the employed consumer that is time-limited<1yr. Service will be
provided to the consumer who is working in an integrated setting or
self-employed setting where the individual is able to interact with non-disabled
persons. It is our goal to support the consumer to be able to develop positive
work habits, attitudes, work etiquette as well as how to become a part of the
informal workplace culture.
We will ensure that we collaborate with the ISP team to identify vocational
outcomes that incorporate progressive moves, that the consumer has the
opportunity to participate in gainful, productive, regular work, that the
consumer is oriented to health/safety aspects of the job, that we identify
necessary supports to maintain employment, provide ongoing job coaching &
monitoring of performance, identify/resolve strengths, barriers/life problems,
that we are respectful of consumer’s wishes as per ISP, assist with learning new
skills for maint/advncmt, assist with hygiene, demeanor & dress expectations,
assist with all expected interaction in the course of employment, intervene with
educating employers & co-workers in the abilities/limitations and assist the
consumer with obtaining job enhancements etc.
When job search is provided we will ensure that we participate with the ISP
team to develop job search strategy, goals, steps & hrs needed to achieve goals,
creation of resume, interviewing skills & develop employment opportunities with
local employers. Service outcomes will reported every six months that reflects
at least 75% of consumers meet goals & report annually that 75% referred for job
search become employed. Qualified staff will be hired/trained to meet the ISP
requirements of the individual. Records will be maintained according to
requirements .

Center Based Employment
We will provide consumers a safe, healthy and supervised work environment in
a setting where the majority of individuals have disabilities and are supervised
by our staff. Consumers are paid according to Department of Labor standards and
are offered opportunity to develop skills, abilities and behaviors that will
allow them to realize their vocational aspirations and support their transition
into a more integrated setting through gainful, productive and remunerative
work.
We will ensure that we have ongoing availability of paid work, that we
develop and implement vocational outcomes according to ISP, that we provide
employment specific training to each consumer, that we incorporate teaching of
general work skills and ethics, that we evaluate job performance identifying
strengths and barriers and develop strategies with the ISP team that focus on
minimizing barriers to success, that we assist with personal care and meals,
that we provide a variety of work opportunities and participate accordingly to
encourage progressive moves through VR referrals.
Service outcomes will reported every six months that reflects at
least 75% of consumers meet goals, 20% demonstrate increased productivity, 20%
have increase in total wages, 10% are identified for progressive moves and 75%
of paid work is available to consumers. A typical work day will consist of
approximately 6 hours with a staff to client ratio of 1:6. Qualified staff will
be hired and trained in developing and teaching meaningful employment related
activities and to meet the ISP requirements of the individual. Records will be
maintained according to Division requirements.

Employment Support Aid
Through this service we will provide to the consumer necessary 1:1 personal
care, behavioral and/or follow-along supports for the consumer to remain stable
in his/her employment setting up to 1-3 hr/day. Supports will be implemented as
per the consumers’ level of need & service setting to accomplish the goal of
empowering the client to attain, maintain or advance in employment.
In the center-based setting we would utilize this service to assist the
consumer with the provision of personal care needs who would otherwise be
excluded from participating. In the group supported setting we would utilize
this service to assist the consumer with personal care needs and/or behavioral
intervention to support ongoing placement in the workplace. In the individual
supported employment setting we would utilize this service for personal care
needs and/or behavioral intervention to support workplace behavior.
Personal care needs may include; assistance with lavatory usage,
mealtimes/breaks, self-medication/reminders, ambulation. Behavioral supports may
include when appropriate; shadowing to assist with the maintenance of
appropriate behaviors in the workplace, support to resolve inappropriate
behaviors, support with resolving personal concerns that may interfere with job
performance, supports to communicate any additional medical or social needs. In
addition, we would utilize this service in the capacity of follow-along for
employed consumers not using any other employment support services as a stand
alone service for the provision of follow-along supports to provide ongoing job
related supports to enable the consumer to sustain his/her job as well as
personal care/behavioral supports as per the employment support aide agreement.
Qualified staff will be hired and trained in developing and teaching
meaningful employment related activities and to meet the ISP requirements of the
individual. Records will be maintained according to Division requirements.
