Direct Support Professional (DSP)
Part-Time & Full-Time CST for Residential Group Home Setting All Shifts
Provide a broad array of support services to promote the physical, emotional,
and mental well being of the consumer. Assist the consumer to acquire knowledge
and skills to be a valued member of his/her community based on his/her own
choices. Provide training and supervision for the consumer to increase or
maintain his/her self-help, socialization, and adaptive skills to reside and
participate successfully in his/her own community. Support the consumer with
developing positive relationships with his/her families. Provide opportunities
for the consumer to interact socially with family, friends, and the community at
large, including facilitating access to community resources. Assist the consumer
in achieving and maintaining a quality of life that promotes the consumer's
vision of the future. Provide assistance and training related to personal and
physical needs and routine daily living skills, implement strategies to address
behavioral concerns, ensure that the health needs of the consumer are met,
assist with the implementation of therapeutic recommendations, assist with
mobility and adaptive training, assist the consumer with medical requirements,
provide general supervision, assist with opportunities for training and/or
practice in basic consumer skills such as shopping, banking, money management,
access to community resources, and community survival skills. Support the
consumer to develop, maintain, or enhance independent functioning skills.
Provide for the consumer's transportation needs. Develop a monthly
on-site/community integrated schedule of activities together with the consumer.
Coordinate with other service providers to ensure continuity of care for the
consumer.

Direct Support Professional (DSP)
Part-Time Housekeeping Position
Routine household activities in the consumer's home. Provide assistance to
the consumer to attain or maintain safe and sanitary living conditions including
but not limited to: dusting; cleaning floors, bathrooms, oven, refrigerator,
windows, kitchen; washing dishes; routine maintenance and cleaning of household
appliances; changing linens; making bed; washing, drying and folding the
consumer's laundry.

Direct Support Professional (DSP)
Part-Time Provider of Habilitation
Providing the consumer assistance with specific training interventions that
are designed to maximize the functioning of the individual in the consumer's
home or community. Interventions are developed in accordance with the consumer's
ISP (Individual Service Plan) and may include but are not limited to:
habilitative therapies, special developmental skills, behavior intervention, and
sensory motor development.

Direct Support Professional (DSP)
Part-Time Attendant Care Provider
Provide consumer with the necessary supports to remain in his/her home and/or
participate in work/community activities. Assist the consumer to develop and
implement an Attendant Care Agreement which will facilitate the consumer with
being able to attain or maintain safe and sanitary living conditions and/or
maintain personal cleanliness and activities of daily living. Assist the
consumer to remain in his/her home and/or participate in community activities.
Provide supports such as: bathing, oral hygiene, toileting, bowel and bladder
care, dressing, shampooing, ambulation, transfers, eating and meal preparation,
routine skin and nail care, repositioning for individuals who are mobility
impaired, assistance with special appliances, shopping, provide appropriate
attention to injury and illness, assistance with self-medication and with
household chores to maintain safe and sanitary living conditions.

Direct Support Professional (DSP)
Part-Time Respite Care Provider
Provide short-term care and supervision consistent with the health needs of
the consumer. Provide relief to family member or person caring for the consumer.
Provide supervision either in or outside of the home, as well as supporting the
emotional, physical, and mental well being of the consumer. Provide care
according to the consumer's plan of care such as: provide for the social,
emotional, and physical needs of the consumer; ensure the consumer receives
medication as prescribed; provide first-aid and appropriate attention to injury
and illness; ensure provision of food to meet daily dietary needs; assist the
consumer with transportation, activities of daily living, employment, medical
appointments, visits with family and friends and other activities, and carry out
any identified programs in the consumer's ISP (Individual Service Plan).
